The revision advances understanding of IIPs and other interstitial lung diseases. In the November-December 2015 issue of RadioGraphics, one of our three articles on chest imaging reviews the 2013 American Thoracic Society-European Respiratory Society update on the classification of idiopathic interstitial pneumonias (IIPs). The authors of this article include radiologists, a pulmonologist, and a pulmonary pathologist who describe the organization of the IIPs into four groups: chronic fibrosing IIPs (idiopathic pulmonary fibrosis and idiopathic nonspecific interstitial pneumonia), smoking-related IIPs (respiratory bronchiolitis–associated interstitial lung disease and desquamative interstitial pneumonia), acute or subacute IIPs (cryptogenic organizing pneumonia and acute interstitial pneumonia), and rare IIPs (lymphoid interstitial pneumonia and idiopathic pleuroparenchymal fibroelastosis). The article includes a table (Table 1) that details the key radiologic features and differential diagnosis of the IIPs. An important component is the list of CT findings such as the presence of cysts, perilymphatic or centrilobular nodules, tree-in-bud opacities, and mosaic attenuation that suggest a diagnosis of a non-IIP entity (Table 3). Idiopathic Interstitial Pulmonary Fibrosis can be classified as multiple diseases including 1. Cryptogenic Organizing Pneumonia (COP) 2. Desquamative Interstitial Pneumonitis (DIP) 3. Usual Interstitial Pneumonitis (UIP) Recently the food and drug administration approved the use of pirfenidone (Esbriet). Most side effects are gastrointestinal like nausea, vomiting, diarrhea, abdominal pain. They can also elevate liver function test and lead to rashes. These patients must be followed closely. In our office, it is diagnosed with multiple tests, a CAT scan, 6 minute walks, and a biopsy. Once we start with the medication, we start with a very low dose. If they tolerate it well then we go to the next dose. We also follow baseline liver function tests. If they become abnormal, they must stop the medication because of the side effects that could be developed. They are also followed with 6 minute walks. It has been shown that pifenidone keeps the patients symptom free for a longer period of time. Hypersensitivity pneumonitis or extrinsic allergic alveolitis is an immune-mediated inflammation in the walls of alveoli and bronchioles secondary to inhalation of certain types of organic dusts including microbes (bacteria, fungi or protozoa, including amoebae contaminating water in ventilation systems), animal or plant proteins, and several low-molecular weight chemicals. It is due to T-helper cell-mediated delayed hypersensitivity reaction to inhaled antigens. T helper cells I (Thl) release tumour necrosis factor, interferon-alpha, IL-12, and IL-18 while Thl7 cell release IL-17 and IL-22.
